If you are seeking a Second Opinion or a Holistic Evaluation for your pet, please inform us when scheduling the appointment to ensure that we schedule appropriately and can review all pertinent information regarding your pet’s condition. If the patient has been under the care of another veterinarian, please have the records and the results of all diagnostic tests sent to us prior to your appointment.

In order to expedite the check in procedure for your visit to our animal hospital, we have provided the following detailed history forms for you to complete before your appointment. We appreciate you taking the time to fill out the appropriate forms and sending them by e-mail ([email protected]), fax (630-737-1332) or mail (1007 Maple Avenue, Lisle, IL 60532) prior to your scheduled appointment. Please provide the information requested as completely and specifically as possible so that we can best serve you and your pet.
